INSURED PATIENTS
We require patients to provide their ACTIVE health insurance ID card/cards and VALID Fl Driver's License or photo ID upon every check-in.
We will bill your visit directly to your insurance payer if you supply an updated and active proof of insurance at each visit. Please note that the insurance policy is between you and your insurance provider, we are happy to file the claim for you. Please ensure that you are providing us with your ACTIVE insurance upon check-in to avoid insurance payment denials and avoid being billed for unpaid services.
Copays, co-insurances are the patient's responsibility and any amount not covered by your insurance policy. Balances are due immediately after receipt of your first invoice.

Our MEDICAID panel is CLOSED. We only accept Medicaid as 2ndary insurance.
MEDICARE & TRICARE
We accept patients on the Medicare & Tricare plans and will also bill your secondary insurance provider directly if any. Patients will be asked to sign an Advanced Beneficiary Notice for services
not covered under Medicare. Patients shall be accountable for covering these services if the secondary insurance does not pay.
As a courtesy and for the convenience of our patients, we offer in-office blood draw service and other diagnostic tests and procedures.
