In order to make sure your visit to the practice goes smoothly, we've put together all the important information about how things work at the practice.
Please present your ACTIVE health insurance ID and valid FL driver's license or photo ID every visit upon check-in.
In order for us to register and treat you promptly, we request that NEW patients bring their ACTIVE health insurance ID, FL driver's license or photo ID on their first visit, together with the completed new patient registration form.
Please also bring a copy of your prior medical records, list of medication, allergy list, most recent laboratory and diagnostic results if any, on your initial visit to the clinic.
Please note that we do not issue health certificates (FMLA, Disability, Employment, etc) and clearances for new patients.
Established patients are required to present their ACTIVE health insurance ID and FL Driver's License every visit upon check-in. We ask that patients immediately inform front desk of changes in their health insurance, address and contact information to ensure demographic information are up-to-date and claims are filed to the correct and active insurance to avoid insurance denial and the patient being billed for unpaid services.
We accept most types of health insurance. If you do not have insurance, please ask us to learn more about our self-pay rates.
Company Protocols & Policies
We do not practice pain management & psychiatry. We will not write nor refill pain & psych medications. Patients will be referred to psychiatry and/or pain management clinics/physicians for their specific condition. If you are taking scheduled drugs, you will be asked to do a urine drug screening test before seeing the provider.
We have a 3 month refill policy. If you have not seen the physician/nurse practitioner for three months or more, you will be asked to consult the provider for an updated evaluation of your medical condition and prescriptions before a refill is authorized. We cannot refill medications prescribed by providers NOT within our practice and cannot refill INactive medications until seen by the provider. Please allow us 2-3 business days to attend to your refill requests. Please allow us 2-5 business days to process a medication prior authorization request.
If you need a specialist referral, we will refer you to our preferred specialists. Please call your insurance to verify if the specialist is IN-Network with your insurance before setting up an appointment. If out-of-network, please ask your insurance to refer you to an IN-Network specialist.
If you need a prior authorization for a specialist visit, please obtain a confirmed appointment with the specialist and have them fax to our office their prior authorization request. Please allow us 2-5 business days to process your request. We do not process same day prior authorization requests.
If you are given a laboratory/ imaging order or requisition, please call your insurance to verify if the lab or imaging facility is IN-Network with your insurance before scheduling an appointment. Please verify your active insurance is correctly written on the form to avoid insurance denial and being billed for unpaid services
If you owe us a balance for more than 30 days, we require that you pay the balance in full before you are seen by the provider. If you are in financial hardship, please ask us regarding our financial assistance payment program.
We will collect $100 for Telemedicine upon check-in since Telemedicine is no longer covered by most insurances except Medicare. Pls call your insurance to verify coverage. We will refund once insurance payment is received.
In consideration of the physician's time and other patients' time as well, beginning May 1, 2012, we will be charging a NO SHOW FEE of $25 for patients who missed their appointment but failed to cancel at least 24 hours prior to their appointment.
